By accepting Summer of the Arts volunteer policies, I agree to and acknowledge the following: All volunteers must be 14 or older to volunteer independently. Volunteers 8 to 13 years of age must have a supervising adult volunteering with them (i.e., parent, guardian, youth group leader, etc.).
Volunteers 18 years or older: As a condition of volunteering, I give permission for the Summer of the Arts to conduct a background check on me, which may include a review of sex offender registries, child abuse and criminal history records. I understand that, if appointed, my position is conditional upon the organization receiving no inappropriate information on my background. This is NOT applicable if the volunteer is a minor.
I hereby give Summer of the Arts permission to use photographs of me as a result of my participation in volunteer programs with the agency. These photographs may be used without identification of me by name in future publicity and communications of Summer of the Arts for an indefinite period of time.
Further, I agree that Summer of the Arts shall not be responsible for any injury to me or damage to my personal property as a result of my participation in volunteer programs with the agency.
